At EOAC, putting people first and acting with integrity means listening when something does not feel right. We are committed to treating every community member with dignity, fairness, and respect and to reviewing concerns responsibly. If you have a concern about your experience with an EOAC program, how you were treated, or whether a process felt fair, we want to hear from you.

This form may be used by program participants, applicants, family members, community partners, or members of the public to share a concern about EOAC services. Your submission will be reviewed and routed to the appropriate staff for follow-up.
